**MANDATORY ATHLETIC PARTICIPATION FORMS - 2023-24**

PLEASE READ AND COMPLETE ALL NECESSARY STEPS NO LATER THAN AUGUST 1, 2023:

To be eligible for competition during the 2023-24 seasons, please complete each section below by AUGUST 1, 2023. You MUST complete each applicable section in order to be cleared.  Thank you for your attention to this important matter.

1. Click to complete your Bulldog Student-Athlete Online Biography Form.

  • This must be completed EVERY YEAR! 
  • You must be logged in to myRedlands in order to access the form, which means you need your Redlands login credentials.
  • Please email Sports Information Director Lance Franey with questions regarding the biography form.

2. MEDICAL HISTORY/INSURANCE INFORMATION

  • All student-athletes (new and returning) must complete a pre-participation physical exam with their respective physician prior to campus arrival.

NOTE: the pre-participation physical MUST be completed on the University of Redlands Athletic Department Physical Form, provided on your SportsWare account, dated after May 15, 2023, and signed by an MD or DO, with his/her respective stamp and their contact information (this is an NCAA requirement).  NO EXCEPTIONS.  Your physical will not be accepted if these items are not correct.

  • All necessary forms and information are posted on a program called SportsWare that you will gain access to by clicking on the link below. Follow all directions on the link to complete the process.  Each student-athlete is responsible for completing all information - if you are under the age of 18, you must have your parent's signature on each form provided.  
  • For first-time account users, you will not have access to the paperwork the first time you click on the link.  You must receive "approval" from the athletic training staff before proceeding.  Please allow 1-2 business days for approval.
  • Choose the link that pertains to you (NEW or RETURNING STUDENT-ATHLETE). This will direct you to the SportsWare access instructions in PDF format.  
    • All forms you need to complete (physical, medication verification, sickle cell, etc.) can be accessed and filled out once on SportsWare. DO NOT SKIP ANY STEPS ONCE YOU ARE FOLLOWING SPORTSWARE INSTRUCTIONS.  If you skip over any steps, the process will be incomplete.

  • EVERYTHING MUST BE SUBMITTED NO LATER THAN AUGUST 1, 2023.  YOU ARE NOT CLEARED FOR PARTICIPATION UNTIL THIS PROCESS IS COMPLETE.

3. Click to complete the NCAA TITLE IX ATTESTATION - this is required annually by the NCAA for ALL student-athletes.  

  • All incoming, continuing, and transfer student-athletes must complete an annual disclosure related to their conduct that resulted in discipline through a Title IX proceeding or in a criminal conviction for sexual, interpersonal, or other acts of violence. 
  • Transfer student-athletes also must disclose whether a Title IX proceeding was incomplete at the time of transfer.
  • Failure to make a full and accurate disclosure could result in penalties, including loss of eligibility to participate in athletics as determined by the member institution.
  • Please read carefully and choose the answers that pertain to your current and previous institution(s), including your high school.
  • You must be logged in to myRedlands in order to access the form, which means you need your Redlands login credentials.

4. NEW TRANSFER STUDENTS (entering Redlands this semester from a 2-year or 4-year institution)

Please request FINAL, OFFICIAL transcripts from all previous institutions be sent directly to the UR Admissions Office AFTER spring grades have been posted. Hand-delivered or in-progress transcripts will NOT be accepted.  If you have NCAA eligibility questions, please email Associate Director of Athletics for Compliance Rachel Roche

5. INTERNATIONAL STUDENT-ATHLETES 

International student-athletes are required to complete an amateurism-only certification account through the NCAA Eligibility Center in order to be cleared for competition at a Division III school.  For this purpose, "international" is defined by the NCAA as meeting any of the following criteria: 

  • Attended high school outside the United States, its territories, or Canada after age 11.
  • Your team was based and competed outside the United States (age 13 or older).
  • Your current address is outside the United States.

If you are an international student-athlete enrolling at the University of Redlands for the first time (first-years and transfers), you must create an account, complete the registration, and request certification before August 1, 2023.  Learn more about this process. 

6. NAME, IMAGE, and LIKENESS Policy - for your information.  The NIL Disclosure form is only available to enrolled and rostered student-athletes.
